Most people think of "working a muscle" as one thing. Lift it, lower it, done.

In reality, your muscles work in three distinct ways, and each one does a different job in your body. Concentric: the muscle shortens as it contracts. Think the "up" phase of a bicep curl or standing up out of a squat. This is the action most people picture when they think about strength.

Eccentric: the muscle lengthens while still producing force, acting as a brake. The lowering phase of that same squat, or your quad controlling your knee as you walk downstairs. Muscles can actually produce more force eccentrically than concentrically, which is part of why this phase gets so much attention in both rehab and performance research.

Isometric: the muscle contracts without changing length at all. A wall sit, a plank, or holding a weight steady in front of you. No movement, just tension.

Every functional movement you do-walking, lifting, running, catching yourself on a curb-blends all three. The question we ask in the clinic isn't "which one is best," it's "which one does this tissue need right now."

Why This Matters in Rehab

Recent research backs up something we see daily in practice: the type of muscle action you choose changes the outcome, not just the exercise itself.

Isometrics calm things down first. For irritated tendons, Achilles, patellar, rotator cuff, tennis elbow, isometric holds are one of the few interventions shown to produce fairly immediate pain relief, likely by reducing inhibition at the level of the motor cortex, not just locally at the tissue. That's why an early-stage tendinopathy program almost always starts here.

Eccentrics rebuild capacity. Once pain is under control, eccentric loading is where tendons and muscles rebuild their ability to absorb force. It's a key driver of collagen remodeling and tendon resilience, and several recent reviews continue to support eccentric-focused programs for tendinosis and chronic tendon pain specifically.

Concentric strength closes the loop.You can't return someone to running, lifting, or sport without their muscles being able to produce force and shorten powerfully again. Concentric work is where we rebuild the "go" after eccentrics rebuild the "control."

This is also why a good rehab program is never just "do these three exercises." It's sequencing the right muscle action at the right phase of healing.

Why This Matters for High-Intensity Exercise and Performance

This isn't just a rehab conversation — it's a performance one too.

Eccentric strength protects against injury. This is best studied in the hamstrings. Programs built around eccentric hamstring work, like the Nordic curl, have been shown in meta-analyses to meaningfully reduce hamstring strain rates in field sport and running athletes, with some pooled data showing close to a 46% reduction in hamstring injury rate. If you're training for obstacle course racing, running, or field sports, eccentric strength is injury insurance.

Accentuated eccentric loading builds power. Because muscles tolerate heavier loads eccentrically, some athletes train with eccentric loads that exceed what they could lift concentrically, think a heavier lowering phase on a squat than they could stand back up with unassisted. Current research shows this doesn't always produce an immediate performance boost in a single session, but consistent use over time is linked to real gains in strength, power, and speed, especially in well-trained athletes.

Isometrics build a stronger foundation. Isometric training at long muscle lengths is gaining traction for building baseline strength and tendon stiffness without the joint stress of heavier dynamic lifting, useful for athletes managing a high training load or working around a cranky joint.

Concentric power is what shows up on race day. All that eccentric control and isometric stability ultimately needs to convert into the ability to produce force quickly, the sprint start, the jump, the pull-up over the wall.

The Big Takeaway

There's no single "best" muscle action. Isometrics calm pain and build a base. Eccentrics rebuild capacity and protect against injury. Concentrics convert all of that into usable strength and power. The skill is knowing which one your body needs and when.
